NURSE AIDE (NA) - ONGOING CLASSES ENROLLMENT

To be admitted:

1. Be able to read, write, and speak English.
2. Be in good health and able to bend, twist, lift at least 50 pounds, and be free from communicable disease.
3. Should not be listed on the Nurse Aide Registry with a revoked status; Cannot be listed as an unemployable on the Employee Misconduct Registry (EMR) and cannot have been convicted of a criminal offense as listed in Texas Health and Safety Code 250.006.
4. Provide a copy of your Driver's License, ID card or Passport. 
5. Provide a copy of your Social Security Card. 

**You will also need to buy a pair of blue scrubs to be worn during class and clinical labs.

**Please submit your Registration Form at the school in person (401 W Bedford Euless Rd, Hurst TX 76053).

**Pay your registration fee of $75 via Zelle - Mamboridge Academy.
